Systemic lupus erythematosus (SLE) can cause systemic complications throughout the body

Anemia. About half of patients with SLE are anemic. Causes include:

Hemolytic anemia can occur with very high levels of the anticardiolipin antibody. It can be chronic or develop suddenly and be severely (acute).

Antiphospholipid Syndrome. Between 34 - 42% of patients with SLE have antiphospholipid syndrome (APS). This is a specific set of conditions related to the presence of autoantibodies called lupus anticoagulant and anticardiolipin. These autoantibodies react against fatty molecules called phospholipids, and so are called antiphospholipids. Their actions have complex effects that include causing narrowing and abnormalities of blood vessels.

• Patients who have APS have a very incidence of blood clots, which most often occur in the deep veins in the legs. Blood clotting, in turn, puts patients at higher risk for stroke and pulmonary embolism (clots in the lungs).

• The effects on blood vessels have also been associated with confusion, headaches, and seizures. Leg ulcers can also develop.

Not all patients with APS carry both of the autoantibodies, and they can also wax and wane and so have varying effects. APS also occurswithout lupus in about half of patients with the syndrome.

Thrombocytopenia. In thrombocytopenia, antibodies attack and destroy blood platelets. In such cases, blood clotting is impaired, which causes bruising and bleeding from the skin, nose, gums, or intestines. (This condition can also occur in APS, but it is not considered to be one of the standard features of the syndrome.)

Neutropenia. Neutropenia is a drop in the number of white blood cells. Patients with SLE often neutropenia, but the condition is usually harmless unless the reductions are so severe that they leave the patient vulnerable to infections.

Acute Lupus Hemophagocyte Syndrome. A rare blood complication of SLE that occurs primarily in Asians is called acute lupus hemophagocytic syndrome. It is generally of short duration and characterized by fever and a sudden drop in blood cells and platelets.

Lymphomas. Patients with SLE and other autoimmune disorders have a greater risk for developing lymph system cancers such as Hodgkin’s disease and non-Hodgkin’s lymphoma (NHL).

HEART AND CIRCULATION COMPLICATIONS

Heart disease is a primary cause of death in lupus patients. The immune response in SLE can cause chronic inflammation and other damaging effects that can cause significant injury to the arteries and tissues associated with the circulation and the heart. Patients with SLE, have a higher risk for developing the following conditions, which put them at risk for heart attack or stroke:

The fibers controlling the lower extremity are posterior to those of the upper limb. Corticospinal fibers traverse the middle portion of the cerebral peduncle of the midbrain and then the basal pons. They enter the pyramids of the medulla (from whence they get their name). Over 90% of the axons in the pyramids decussate just before reaching the upper cervical spinal cord (the pyramidal decussation) and they enter the lateral funiculus of the spinal cord to become the (lateral) corticospinal tract . Most of these axons terminate in the intermediate gray matter of the cord, although some enter the dorsal horn (where they can have an effect on sensory transmission) and a few terminate directly on alpha motor neurons, contributing to rapid voluntary movement. Most of these fibers terminate on interneurons of the spinal cord. These interneurons are responsible for reflexes and, therefore, most motor activity actually occurs by the regulation of reflex excitability in the spinal cord.

A few corticospinal axons descend the anterior funiculus of the spinal cord as the anterior (ventral) corticospinal tract. This is more involved in axial (trunk and neck) movements and terminates bilaterally.

Corticobulbar

Many projections from the cerebral cortex terminate in the brain stem (generically called corticobulbar projections). These projections have several functions including voluntary control over cranial nerves, relay to the cerebellum, activation of other descending pathways (i.e., "indirect corticospinal projections") and modulation of sensory processing.

Many cranial nerve nuclei receive direct and indirect (through the reticular formation) cortical input via nerve fibers arising from the motor cortex and traversing the genu of the internal capsule. Most corticobulbar connections are bilateral, meaning that unless both sides of the nervous system are affected, there is no loss of motor control. However, the facial nucleus to the lower face receives only input from the contralateral motor cortex and, therefore, there will be weakness of voluntary movement of the lower face on the side opposite damage to corticobulbar neurons (with sparing of movements of the upper face).

The majority of corticobulbar projections terminate in the ipsilateral basal pontine nuclei. These nuclei relay to the cerebellar cortex via projections that decussate in the pons and enter the cerebellum through the middle cerebellar peduncle (see below). These represent, by far, the largest input to the cerebellum.

Bulbospinal projections

There are several brain stem nuclei that project to the spinal cord. The cerebral cortex projects to most of these and, therefore, may affect them as "indirect corticospinal projections." These areas include the red nucleus, which gives rise to the rubrospinal tract that decussates in the midbrain and descends the lateral funiculus near to the location of the lateral corticospinal tract . The reticular formation gives rise to several descending pathways, one from the rostral pons that helps pattern locomotion, one from the caudal pons that can affect head movement to coincide with eye movement and one from the medulla that mostly inhibits reflex activity in the spinal cord. This latter tract is excited by cortical input and, therefore, cerebral motor cortical output is mostly inhibitory to spinal cord reflexes via this indirect pathway. For this reason, interruption of corticobulbar projections typically increases reflexes.

Cerebral cortical projections also go to the superior colliculus, a region that gives rise to a tectospinal tract as well as projections to eye movement centers. The superior colliculus is mostly responsible for reflex head and eye movement toward novel stimuli. The cerebral cortical projections to the superior colliculus may effect movement via these projections.

Babinski reflex: An important neurologic test based, believe it or not, upon what the big toe does when the sole of the foot is stimulated. If the big toe goes up, that may mean trouble.

The Babinski reflex is obtained by stimulating the external portion (the outside) of the sole. The examiner begins the stimulation back at the heel and goes forward to the base of the toes.  There are the things you can’t ignore—bad breath, yellow teeth, stained fingers and lips, age spots, and wrinkles. Then there are the things you cannot see—the steep increased risk for lung cancer, emphysema, fertility problems, heart disease, and stroke. If you are a smoker who is thinking of quitting or has tried to quit before, chances are you already know all the reasons why you should.
Who Smokes?
Smoking crosses all racial, gender, and socioeconomic lines. According to The Centers for Disease Control and Prevention (CDC), more than 46 million American adults smoke—more than 20 percent of the adult population. (That is down from 24 percent 10 years ago.) More men light up than women—23 percent of men compared to 19 percent of women. And it is not just adults. Nearly a quarter of high school students in the U.S. smoke cigarettes.
What is in a Cigarette?
Cigarettes are made with dried tobacco leaves that naturally contain the drug nicotine. Cigarette manufacturers add chemicals like ammonia, tar, lead, and cyanide and other ingredients, like cocoa, coffee, and menthol, to change the flavor of the tobacco in an attempt to make smoking more enjoyable. According to the American Cancer Society (ACS), more than 4,000 different chemicals have been identified in cigarettes and cigarette smoke. Of those 4,000 chemicals, 60 are known to cause cancer. These cancer-causing chemicals are called carcinogens.
Until recently, cigarette companies were not required to make public a list of the ingredients that were added to their cigarettes, so scientists have been unable to determine what, if any, effects all these additives might have on a person’s health. Recently passed federal legislation now requires cigarette manufacturers to provide the Food and Drug Administration (FDA) with a list of additives in their cigarettes. A list of harmful or dangerous chemicals found in each cigarette will be made public for consumers by or before June 2011. For the most part, tobacco products are unregulated—unlike medications, spirits, and most foods—which means their potential health risks, beyond what we already know from years of consumer use, are still being discovered.
Smoking, Pregnancy, and Fertility
Smoking and tobacco use affect reproduction and fertility. Research shows that men who smoke have lower sperm counts, and the sperm they do have is often misshapen and has a harder time moving—making conceiving more difficult. Experts also believe smoking affects sperm DNA which may lead to developmental and physical health problems in a child.
For women who are pregnant and still smoking, you are damaging the health and future health of your unborn child. Babies born to mothers who smoke are about 30 percent more likely to be born prematurely, and those who do make it full term are more likely to have a low birth weight. Babies born to mothers who smoked during pregnancy are also at a greater risk of sudden infant death syndrome (SIDS).
Smoking and Disease Risk
The leading cause of death in American men and women is heart disease. Smoking increases your risk for heart disease two to four times compared to a nonsmoker. The same thing goes for your risk of stroke. Smoking narrows your blood vessels and arteries. This may lead to peripheral artery disease (PAD)—the obstruction of the large arteries in your arms and legs. If you continue to smoke, PAD can cause a range of complications including pain, muscle determination, and eventually muscle death.
Smoke damages your lungs and your airways, putting you at greater risk for respiratory disease. In men, the risk of developing lung cancer increases 23 times if you are a smoker. Lung cancer is the leading cause of cancer death in women, and smoking increases a woman’s chance of the disease 13 times that of nonsmokers. Most smokers will also develop a chronic obstructive pulmonary disease (COPD)—one of a few diseases, like bronchitis or emphysema, that block airflow into and out of your lungs, making breathing increasingly difficult.
Smoking and Death
Smoking remains the number one cause of preventable disease and death in American adults. In addition, second-hand smoke kills tens of thousands of people who never took a single puff in their lives each year. On average, smokers die 13 to 14 years earlier than nonsmokers. If you don’t quit, you could be one of the estimated 443,000 adults who die each year from a disease or complication attributable to smoking.
Here’s some good news, though: The moment you are no longer a smoker, your risks for many diseases and health complications begin to fall. In fact, according to the American Cancer Society, one year after you smoke your last cigarette, your increased risk of heart disease is reduced by half. Fifteen years later, the risk for heart disease is similar to that of people who’ve never lit up. The same is true for the risk of stroke. Your health and the fate of your health are not set in stone. You can change your future by making the decision to quit.
